What to clarify before discharge

Prepare the home for the first week

Recovery is easier when the home is set up before the client arrives. Clear walking paths, reduce fall hazards, place essentials within reach, and think through bathroom safety, meals, and overnight needs ahead of time.

Families also do better when they decide early who is handling calls, medications, appointments, and hands-on support.

Support that often helps most after discharge

• Post-hospital recovery care for transitions and daily oversight
• Personal care for bathing, dressing, transfers, and routine help
• Nursing support when clinical follow-through matters at home
• Respite support if one family member is doing too much alone

Reduce the risk of a difficult recovery
